For the past month, I've been paying for my health care and dental. They said they would send it to me in the mail (never did). I told them I paid 3 times over a month. They said we have no record of that, you can pay again or go to the hospital without insurance. So basically I'm out about $350, and if I don't get this resolved soon I'm not going to go back for more.

you forgot to keep record of proof that you payed for insurance

it doesn't matter, it's not their responsibility to keep track of what you have paid for
you do the job of keeping track of those things, you're supposed to have a paper trail if you have actually made those payments

and if you don't, or you somehow lose those documents, then you will be left with no proof that you did indeed pay for said services

no they weren't, and even if they had, that is no excuse to say that the person who pays for services is supposed to be responsible for keeping records that said services were paid
you are the one who pays them, if you want your services, you have to pay for them, you are the one responsible if you end up not getting your service, not the company itself
