It's a reference to internet culture, specifically a joke or viral post on the Internet. Think of the 2013 "Doge" meme, or the "cancer kid" meme. Or even the "pepe" meme.

Well, yes, but not really. Memes can be a lot more serious than that, but they're all jokes or posts that are referenced to the internet by someone. That's why you might hear some people saying "I don't get it", because they're unfamiliar with it and don't know the backstory.
