What if he was also a priest in their small town, and is known as the one who saves lives? He is a very kind man, and even takes care of old ladies, and has donated half of his life savings to charity. He's a really loved guy.

I would probably confront him with it, and tell him I know what he did.
I'll let him think about it for awhile, then ask him again.
Then if he continues to lie about it, I will definitely have to get the authorities involved, because I don't think there are many people who are willing to sacrifice their life to save other people.

He'd be like "Yeah, I just couldn't handle it anymore! You're all fat, and your parents were terrible!"
He would obviously try to put blame on me, so I wouldn't say anything.
I'd pretend that I wasn't aware of what had happened.
