You shouldn't need to talk before. Just do it when you need to, it's not that hard.
If it hurts, then she either isn't ready for it yet, or you just aren't doing it right.
If you're doing it wrong, it may be due to lack of foreplay.
Foreplay is a major part of it, and that's why it's recommended you do it when you can take the time needed to pleasure each other. But if you're both too impatient, then just do it.

It still needs foreplay though. The more the better. It doesn't have to be anything fancy, but it has to have some type of build up to it. Otherwise it won't be enjoyable.

It depends on how much time you got, honestly. It could range anywhere from five minutes up to twenty, with the ideal being fifteen. Though that can vary depending on what both of you like. I'd recommend ten to fifteen minutes though.

Ten minutes will give you enough time to last at least twice, which should be good enough to satisfy both of you. If you're more than willing to sacrifice quality for quantity, then yeah, fifteen minutes is probably as much as you can get without making her feel uncomfortable.
