Just like the first step to any relationship, you must introduce yourself. The best way is to say hello and then ask her name. Then follow with something else, but don't do it too quickly. Here are some examples:
"Hello there! I'm Bob, and you are?"
"Hey! I'm John. What's your name?"
"You're new here. My name is Tom."
Then once she answers, introduce yourself again and ask her for her phone number.
"I'm Bob, and you're Tina. It's nice to meet you. Would you mind if I gave you my number? That way we can stay in touch."
"Yes, sure!"
"Don't forget that my number is (555) 123-4567"
You can also just ask if she has a cell phone by saying "Do you have a phone?" or "Do you have a cell?"
You've just got her number!
If you want to take it further, you might want to call her. Say you'll be at lunch on Tuesday, so maybe you could join you. If she says yes, then make plans to see her again.
And then keep doing that.

To get a guy follow the same instructions but just replace his/her with his and hers. Also use his instead of her in the first example.
