If you say "I'm not hungry", does it mean the same thing as saying "I don't want to eat"?

I asked this because I have a friend who is always saying things like that, and he's been making fun of me for liking food too much. He says "I'm not hungry" when I ask him if he wants anything to eat, but I know he's lying.
